Healthcare for the Homeless Program
The LifeSpring federally qualified health center program carries a special designation, known as “Healthcare for the Homeless.” This allows us to provide comprehensive primary care services to homeless patients, those unstably housed, and those at risk of homelessness, at low or no cost. Our healthcare for the homeless program ensures that no patients are without the necessary primary care and patient centered medical home support needed to lead a healthy life. PATH Program
The Projects for Assistance in Transition from Homelessness (PATH) program provides services to people with serious mental illness, including those with co-occurring substance use disorders, who are experiencing homelessness or at imminent risk of becoming homeless. To be eligible for the PATH program, an individual must be 1) determined to be experiencing serious mental illness or co-occurring serious mental illness and substance abuse disorder AND 2) experiencing homelessness or at imminent risk of homelessness. Services available include case management, mental health services, alcohol and drug treatment, service coordination, referral services, benefits acquisition, and representative payee service. Permanent Supportive Housing Program
LifeSpring’s Permanent Supportive Housing Program is a HUD funded program that provides housing to individuals and families who are literally homeless and have a documented disability. Each household has a case manager that works with them to meet the needs that are identified through an assessment. These needs may include financial management, employment, education, child care, mental health services, substance abuse services, medical care and daily living skills.
